Patient Information

Initial Evaluation

I will meet with you and your child for the first visit. As an integrative provider this visit will go well beyond the typical medical assessment. We will review your child’s strengths and where there may be imbalances. Please contact me to discuss how I help.

Follow-Up Visits

The first follow up visit will be scheduled 2–4 weeks after the initial visit. Then it is recommended patients return once a month for the first 3 months. We can then space out appointments as deemed best for your family and child.

Policies

This is what is called a fee-for-service practice. I do not accept insurance, but I do provide an invoice so you can bill your own insurance.

Payment must be made at the time of the visit. I accept check, Visa, MasterCard and American Express.

Changes to scheduled appointments require 48 hours advance notice.
Failure to keep a scheduled appointment without a 48-hour notice will result in a charge for the full fee of the scheduled appointment.

Following your visit, I will be available to you by email from Monday to Friday from 8am to 5pm. I will respond to email within 24 hours from Monday to Friday. If you send an email on Friday afternoon you may not receive a response until Monday morning. In case of an emergency you should call 911 or have your child seen at University of Minnesota Masonic Children’s Hospital.
